Criminal Background Check for Youth Coaches/Volunteers

All coaches or volunteers who have direct contact with youth age 17 and under during club practices or activities must:

1. Have a current USATF membership, and
2. Complete a Youth Coach/Volunteer Application .

Please mail the completed form above, along with a copy of a government-issued ID, to: Patti Petesh, 100 Dexter Avenue North, Seattle, WA 98109.
